Physical Examinations

 

 

The Pennsylvania School Health Law requires that children attending school in the Commonwealth receive a medical examination at stated intervals. 

A medical examination is required on first entry into school (Kindergarten or Grade 1) and in grades 6 and 11. These grades were selected because they represent critical periods of growth and development in a child’s life. Transfer students and students with incomplete health records shall also be required to have a medical examination. 

You have the choice of having this examination done by your family physician or the school physician. To clarify any question concerning the cost of this physical examination, the examination given at school will be done at the cost of the school district, while those done by the family physician will be done at the expense of the parent.

A physical done no earlier than 10 months before the first day of the school year is acceptable.

Please return the completed form by October 15th.

 

If the Private Physician’s Report form is not completed and returned to the school nurse, your child will be scheduled for a physical examination by our school physician during the school term.
